The structural test: A structural change is running when it would still happen if the leader forgot about it. An intention requires the leader to remember. A structure changes the environment. This log tracks structures, not intentions — evidence of what ran, not what was planned to run.

Log discipline: Each entry requires specific evidence — not "I think it ran" but what happened, who was involved, what was said or documented. Vague entries indicate the structure is nominal, not active.
Part 3 — Day 60 Readiness Assessment
Complete at end of Phase 2 — input for Day 60 Psych Safety Check
Day 60 gate: A structure that ran for 4 weeks under facilitation but drifted in weeks 5–6 is not embedded. The readiness assessment asks whether the structure will continue to run in Phase 3 without direct accountability prompts.

What ran as a genuine structure vs. what remained an intention
Name specifically which changes became structural defaults and which ones still depended on the leader remembering to do them.
Where drift appeared — and what caused it
For each week where a structure was rated Partial or Missed — what specifically caused the drift? External pressure? Schedule conflict? The structure was never genuinely structural to begin with?
What Phase 3 needs to embed that Phase 2 did not
One specific structural change that belongs in Phase 3 (Cultural Integration) based on what the log revealed about what is still intention-dependent.
